```Duties```
- Conduct patient assessments to determine their needs and develop individualized care plans
- Coordinate and monitor the delivery of healthcare services to patients
- Collaborate with healthcare professionals to ensure continuity of care
- Provide education and support to patients and their families
- Maintain accurate and up-to-date medical records
- Administer medications as prescribed by healthcare providers
- Advocate for patients' rights and ensure adherence to HIPAA regulations
```Requirements```
- Valid state nursing license (LVN)
- Experience working in case management or a similar role. Hospice experience preferred.
- Knowledge of medical terminology and procedures
- Familiarity with electronic medical records systems
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Experience working with seniors is a plus
- Ability to provide care in various settings, including hospice care
