The Case Manager provides community-based case management for individuals diagnosed with a mental health and or substance use disorders. The Case Manager coordinates care for an assigned caseload by collaboration with a multi-disciplinary team. Population served includes individuals with a mental health and/or substance abuse diagnosis.
Education, Licenses, Certification, Skills and Experience Requirements:
- Minimum Education:
Bachelor's degree in Human Services related field earned from an accredited college or university.
- Minimum Work Experience: One to three years' experience in case management
- Driving Requirements:
Valid unrestricted driver's license and appropriate auto insurance coverage as required by North Central Health Care.
Must have the use of a personal vehicle.
- Preferred Qualifications, Skills, Education, Licenses, Certifications and Requirements:
Three to five years' experience in case management
Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
Essential Job Duties and Responsibilities:
- Determine initial and ongoing eligibility for Community Treatment programs.
- Facilitate admission of consumer to appropriate Community Treatment program.
- Assess the strengths, needs and barriers of individuals.
- Provide program orientation to consumers and multi-disciplinary team.
- Facilitate the service planning process and the identification of consumer needs, goals, objectives, strengths, barriers, interventions, and their priority in the treatment planning process.
- Assess ongoing needs, including crisis situations, to determine treatment interventions and service provision.
- Provides services in adherence to the pillars of Community Treatment program philosophy which include being strength-based, recovery-oriented, person-centered, flexible, trauma informed and consistent with best practice.
- Completes documentation in compliance with Community Treatment standards, agency policy and applicable
- Adheres to scheduled work hours with regular and predictable attendance.
