Near North Health, a longstanding pillar of Chicago's healthcare landscape, is seeking a visionary and compassionate Nutritionist to join our dedicated team. Since our inception in 1966, we have remained committed to our mission of providing accessible, affordable, and high-quality primary healthcare services to all, regardless of their financial circumstances. 

As Nutritionist, you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your expertise in providing participant centered nutrition education, contributes in the effort to achieve objectives and will further solidify our position as one of the largest providers of community-based primary care in the city. 

 Key Responsibilities:

• Caseload Management: Assists all WIC Program staff in meeting assigned caseload set for each fiscal year.
• Meeting Program Goals: Assists all WIC Program staff in meeting the program goals and objectives set for each fiscal year. (e.g. nutrition education, breastfeeding, immunization, Family Case Management integration, etc.)
• Counseling: Conducts nutrition assessments and provides participant centered nutrition counseling to WIC participants. Serves as a model of all staff of exemplary participant centered education.  Formulates care plans and documents interventions using the SOAP format.
• Provide Benefits: Approves appropriate formulas and foods and follows up with the healthcare provider when needed.
• Education: Provides secondary nutrition education contacts; individual or group sessions, and/or food demonstrations.
• Promotion and Support: Promotes breastfeeding, provides breastfeeding support and education; including pumps, referrals and incentives

Qualifications

• Graduation from an accredited college or university/Bachelor’s Degree with a major in foods and nutrition. 
• Knowledge of and skilled in principles of participant centered education/services and behavior change theories
• Ability to work well with people of varied cultural and socioeconomic backgrounds
• Ability to establish and maintain effective working relationships with employees, other agencies and the public 
• Performs clerical work in the absence of clerical staff
