- Purpose of Job
- Create and maintain relationships with referral sources in order to generate new business and develop community standing. Coordinate referrals from all service lines and provides support to new and existing clients. Assist in coordinating staffing for client visits and admissions.
- Job Requirements
- Graduation from an accredited School of Nursing required.
- Bachelor's of Science in Nursing (BSN) preferred.
- Current valid Registered Nurse (RN) License, valid compact multistate license to work in Nebraska or Iowa (work state), or a temporary permit while awaiting licensure required.
- Must maintain current driver’s license and auto insurance coverage within the limits specified by MFH’s liability insurance carrier and stated in the MFH employee handbook.
- Must provide proof of license and insurance upon request and have access to reliable transportation.
- Must maintain an acceptable driving record as set forth in MFH’s policies and procedures.
- Current American Heart Association Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
- Minimum 3 years clinical experience required.
- Home Health Care experience preferred.

Hazards - Essential Job Functions
- Work independently to triage calls and coordinate referrals from all MFH service lines, and provides support and coordination of service to new clients as well as to existing clients.
- Obtain and verify third-party policy information with the patient and/or referral source.
- Gathers core clinical records necessary for insurance authorization; i.e., current diagnosis, medications and H&P, face-to-face documentation.
- Documents appropriate referral information Supports agency financial and quality goals by completing and submitting required documentation within 24 hours of services provided.
- Assesses the needs of clients referred to Home Health Care and Hospice determining appropriate service, department and discipline based on regulatory guidelines and funding.
- Coordinates and facilitates new referrals and re-admissions related to post-facility stays.
- Advises physicians, nurses, social workers, clients and their families on policies regarding admission criteria, and obtains physician orders for initiation of services as required.
- Communicate and coordinate with management about a potential client’s clinical condition, those requiring immediate attention, those with unique needs, and those with complex treatment plans.
- Documents doctor’s orders, lab notes, case conference notes and other information into the computer.
- Facilitates positive outcomes by providing education regarding client’s role in working toward independence.
- Provides information about Home Health Care and Hospice services to patients, families and referral sources including funding options and community programs.
- Triages and responds to calls concerning all Home Health Care and Hospice clients to include intermittent home care, maternal/child, mental health, health services, hospice, pharmacy and IV departments and service lines.
- Effectively communicates/coordinates incoming patient information to the appropriate staff to ensure a customer-focused environment for both internal and external customers.
- Understands integrated delivery systems and communication systems.
- Fully engages referral sources by way of marketing and business development.- Coordinate business development and marketing meetings with referral sources.
- Ensure referral sources have marketing items and information to promote MFH HHC and Hospice.
- Responsible for introducing the HHC and Hospice business to new and existing providers and referral sources to strengthen relationships and promote business.
- Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.
