Provide high quality social services support to referred patients in a timely manner, and in line with Neighborcare’s current scheduling expectations. This includes assessing needs, assisting individuals to access and navigate community services, and sometimes following up to ensure that services are received. Examples include, but are not limited to:
- Housing and emergency shelter resources
- Clothing, food and hygiene resources
- Medical appointments and specialty referrals
- Referrals to community behavioral health resources
- Assisting with Application for appropriate benefits, e.g. DSHS programs, Social Security, Medicare, etc.
- Parenting support services
- Transportation services
- Older adult services
Maintain updated knowledge of community resources and organizations that provide needed services to our patients.
Maintain updated knowledge of entitlement programs, including DSHS, Medicaid, and Social Security and their requirements.
Develop and maintain relationships with outside service providers and represent Neighborcare Health’s mission and programs in a professional manner.
Coordinate care with staff at community agencies to facilitate exchange of information on services and shared patients.
Work within role as a mandated reporter of abuse, abandonment, neglect and financial exploitation with identified patients to Child or Adult Protective Services and maintain follow-up as needed.
Work cooperatively as a member of the clinic health care team, including involvement in case consultation, and participation in ongoing coordination of treatment with the primary care medical, dental, and behavioral health teams.
Work cooperatively as a member of the broader Primary Care Social Work team, including information sharing and assistance in covering patient care duties during times of other team members’ leave.
Work cooperatively with clinic reception and other support staff to maximize patient access to social work services
Maintain patient electronic medical charts in accordance with the organization’s policies and procedures, including appropriate use of Disclosure/Consent forms, documentation completed within 72 hours of visit
Attend meetings as directed by supervisors
KEY SKILLS, KNOWLEDGE & ABILITIES
Ability to communicate and collaborate effectively within multidisciplinary medical teams
Demonstrates ability to efficiently and effectively provide social service support to patients with complex psychosocial and medical presentations
Demonstrates independence in utilizing information supports (e.g., internet search tools, community contacts, peer consult) to problem-solve and address patient resourcing needs
Appropriately consults with colleagues or supervisor to problem-solve practice-related questions or issues
Ability to communicate effectively and respectfully with persons of varied racial, educational and socio-economic backgrounds as well as individuals with disabilities
Ability to utilize interpretation services to provide care to non-English speaking patients
Ability to maintain appropriate professional boundaries with patients, families, co-workers and community resources
Knowledge of and ability to maintain professional standards and follow the policies and procedures of Neighborcare and of state, local and federal regulators
Knowledge of and skill in the use of computers with the ability to learn and use required software programs, including clinical documentation in an electronic health record program
Ability to meet and comply with HIPAA/Confidentiality policies and procedures
Education Requirements:
- Masters in Social Work or Bachelor's Degree in related field and a minimum of 2 years of social services experience
