HOUR AND ANNUAL SALARY RANGE: Hourly Salary Range: $27-$30.39; Annual Salary Range: $56,376-$63,454; Non-exempt; Full Time, 40 Hours/Week, Fringe Benefits package at 30%, 1.0 FTE Direct Services
POSITION PURPOSE: The Registered Care Coordinator provides clinical coordination, rehabilitation, and recovery services primarily to adult clients, ages 18 and older, with severe mental illness on an outpatient basis, including adults with co-occurring disorder, and performs related work as required. Will conduct groups and individual sessions with clients.
REPORTING RELATIONSHIP: This position reports to the Program Director
KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge of bio-psychosocial assessment, group and/or individual counseling, medication management, crisis prevention and intervention, rehabilitation, community integration, and other support services to maintain clients least restrictive environment;
Knowledge of California’s state-run and funded health coverage programs and billing requirements
Knowledge of clinical assessment of clients with severe mental illness, counseling theory, and clinical care coordination principles and best practices;
Knowledge and familiarity with CERNER EMR;
Ability to operate office equipment and use of Microsoft Office (e.g., Microsoft Word, Microsoft PowerPoint, Microsoft Excel);
Ability to plan and organize efficiently and demonstrate positive, professional, and tactful communication skills.
