Nemours is seeking a Social Worker (MSW) to join our Ambulatory team in Pensacola, FL!
This position is responsible for assisting the patient and family in adjusting to the child’s diagnosis and treatment plan and helping the patient and family to function optimally throughout the treatment process. Also assists the family in maximizing utilization of the health care system and links them with the appropriate services necessary to meet their needs.
The Social Worker will serve as the psychosocial leader on the treatment team, providing psychosocial assessment and recommendations for patients. Consults and collaborates with interdisciplinary colleagues involved either directly or indirectly with the case and treatment of patients and their families. Collaborates with community organizations and businesses including schools, parent groups, non-profit organizations and financial institutions regarding childhood illnesses and their impact on patients and families.
The incumbent will assess and provide information and facilitate referrals to community, private, state, and federal agencies based on psychosocial needs.
Essential Functions:
- Conducts psychosocial assessments and depression (PH-Q)/anxiety (GAD-7) screenings. These assessments should identify patient and family strengths, coping styles and problem areas within the context of their diagnostic, social, financial, cultural, and psychological situation.
- Assists families in integrating the medical, social, and emotional dynamics of the disease/treatment into the context of their current life situation.
- Documents social work interactions in the EMR in a timely manner.
- Coordination and linkage with community resources and agencies relative to health, crisis intervention, disability, schools, etc.
- Provides support for patients and their families assisting with emotional, personal, psychological, and socioeconomic factors along with adjustment to illness issues. This includes the entire spectrum from diagnosis, throughout the treatment phase, as well as the bereavement process, if necessary.
- Provides ongoing coordination of services to assist children with long term chronic illnesses in managing the impact of the illness through the various phases of childhood, adolescence and/or transition into adulthood.
- Responds to the needs of culturally diverse and socially, intellectually, or physically challenged patients.
- Provides consultation for physicians and other members of the multidisciplinary healthcare team to address psychosocial needs in the context of adjustment to the illness, treatment, and treatment adherence.
- Establishes and maintains appropriate interpersonal boundaries with patient and family.
Job Qualifications:
- Masters Degree in Social Work
- 1-3 years previous experience in hospital or healthcare setting is preferred
