For this position, we are looking for a US-based LPN or MA. The CCM Program at Neolytix is rolled out at multiple medical practices nationwide. The Care Coordinator position is a vital member of a cross-functional care team driven by their mutual devotion to serving others. Client’s Practice Management leans on this team to supplement the care they cannot provide themselves. This individual will have a team who will be working on patient care coordination that is supplemental to Chronic Care Management of the Client’s Practice. The team aims to maximize care for the Client’s Practice.

Job Description
CCM Responsibilities
- Onboard new practices and operationalize a care plan for their patients.
- Carry out care coordination alongside the practice and perform Chronic Care Management by overseeing care plans determined by the practice.
- Act as primary contact for patients to build rapport to maintain patient satisfaction and improve health status.
- Escalate cases which require provider attention.
- Document assessment data, education provided, and lifestyle interventions planned in patient charts.
- Apply nursing processes to actively manage members and achieve integrated health services through coordinated service efforts.
- Possess ability to change workflow and processes as requested by provider or practice.
- Expectation to have some familiarity with CCM platform and program requirements.
- Maintain performance expectations for CCM team members.
- Contribute as a positive member of the CCM team by supporting team members of the team in a productive and constructive manner.
- All other duties, as assigned.
Required Qualifications:
- Medical Assistant or LPN with Active License/Certification and any necessary license requirements per state regulation as an LPN, Compact license is preferred.
- Must possess advanced telephonic and virtual communication.
- Must have reliable high-speed internet.
- 1 year of relevant experience, preferably in remote chronic care/patient monitoring knowledge and skills
- Comprehension and communication skills with commitment to provide uninterpreted services and the ability to work remotely and effectively to receive and convey information.
- Organization skills including prioritizing, scheduling, time management and meeting deadlines.
- Maintain patients’ privacy, confidentiality and adhere to ethical and legal requirements.
Preferred Qualifications:
- Experience working with computer digital platform/Electronic Medical Records
- Ability to build relationships and earn trust with patients, families, and healthcare providers
- Experience with telephonic triage/physician communications
- Provide autonomous decision-making, troubleshooting, and problem-solving related to periodic system issues
- Knowledge of various software applications such as Microsoft Excel, Word, Outlook
Job Type: Full-time
Pay: $15.00 - $18.00 per hour
Expected hours: 8 per week
