NET Community Care was created to implement the first “Community Umbrella Agency” (CUA) in the city of Philadelphia. Selected by the Department of Human Services to launch a major transformation of child welfare services, NET Community Care is committed to the goals of Philadelphia’s “Improving Outcomes for Children” initiative. We believe that positive outcomes are achieved when services are family-centered, community-based, culturally competent, integrated, timely, and accountable for results. This was and is perhaps the most significant systems change effort Philadelphia has undertaken in decades. The key components or services offered at CUAs include case management, community engagement, parent cafes, team conferencing, and Community Advisory Boards (CABs).

Position: Aftercare Worker

Hours: Full Time, varies depending on Caseload

Location: CUA 1 (25th Police District) other locations as assigned

Feltonville, Hunting Park, Upper Kensington, McGuire, Fairhill, Hartranft and Franklinville

SUMMARY:

The Aftercare Worker works to support families who have been successfully discharged from DHS/CUA formal case-management services by providing reintegration supports, consultations and community resources that will decrease the probability that the family will re-enter the child welfare system. This position involves working closely with all members of the assigned case management team prior to successful discharge; with children, biological families, and with external agencies and partners. The Aftercare Worker will focus on planning intended to prevent re-entry. Tasks will include direct work with families to address needed ongoing community supports after formal DHS/CUA case-management services are discharged, as well as work with external partners and systems to ensure that children and families are successfully connected to needed reintegration and support services..

ESSENTIAL POSITION RESPONSIBILITIES: (and other duties as assigned)

· The CUA must offer/provide Aftercare Services to families who achieve reunification, PLC, or the successful completion of In-home Services.

· Aftercare Services must connect the family to community resources that will support the family’s progress and stability after completion of Aftercare Services.

· Assist with the development of aftercare plans for all families accepting aftercare services and/or transition plans for children stepping down from congregate care to In-Home services.

· Occasionally attend court to assist Case Manager with presentation of intended aftercare/transition plan(s), but never in lieu of Case Manager.

· Attend all Closing Conferences for assigned cases.

· Work on specific skills with the bio-parent(s)/reunification resource(s), including parenting skills, discipline practices, self-advocacy skills, basic budgeting and home-making skills, self-care skills, system navigation skills, and other skills that might help sustain enhanced protective capacities and prevent re-entry to care after safe case closure.

 

MINIMUM QUALIFICATIONS:

· Bachelor’s Degree in Social Work or related field required; Candidates with a degree in an unrelated field require a minimum of three years’ experience working in child, youth, and family serving systems

· Two years’ experience working in child, youth, and family serving systems

· Demonstrates good verbal and written communication skills

· Adult and Pediatric CPR Certification Required

· PA Child Abuse Clearance required

· PA Mandated Child Abuse Reporter Training Required

· FBI Clearance required

· PA Criminal Clearance required

· Health Assessment required

· TB Test required
