Care Coordinator Supervisor

Key Areas of Responsibility:

• Care Coordinator Team & Operations Team Management
  • Recruitment and Hiring for Care Coordinators
  • Provide Training and Competency Assessment for all Care Coordinators
  • Create the CC Schedule and Adjust the Schedule as needed
  • Responsible for always maintaining CC Schedule coverage
  • Maintain Employee Time and Attendance Records including Approvals, Denials and Edits
  • Complete all Performance Documentation per company policy for the CC team
  • Recruitment, training, scheduling and oversight for the ISL House Manager position
  • With the assistance of direct supervisor, create and maintain auditing systems within the Operations Department to ensure compliance with organizational standards, as well as those of our licensing and accrediting bodies
• Clinical and Group Coordination
  • Responsible for tracking attendance in groups.
  • Responsible for tracking that all non-clinical groups have been documented correctly and in a timely manner by CC staff in the EMR
  • Responsible for gathering the clients in treatments at the time of group
  • Responsible for monitoring clients in treatment and ensuring that CC staff communicate concerns to the Clinical Team when applicable
  • Responsible for supervising chores and conducting room checks
  • Responsible for acting as a model and mentor to the clients in treatments
  • Perimeter checks
• Oversight of Med. and UA Compliance
  • Responsible for supervising medication times as per the Medication Policies and Procedures
  • Serve as the liaison between the CC and nursing team
  • Coordinates cross training of CCs in medication management policies and procedures
  • Responsible for collection of UA drug screens and recording the results
• Transportation
  • Responsible for scheduling, staffing and coordinating the transportation needs of clients in treatment to approved appointments, outings, pick-ups, drop offs, etc.
  • Responsible for scheduling, staffing and coordinating the transportation needs of clients at the ISL to and from the Outpatient facility
  • Responsible for ensuring that team members obtain and maintain the appropriate licenses to operate company vehicles
• Compliance Oversight and Document Management
  • Responsible for completing and filing required forms (i.e. Hourly Checks, Shift Change, Weekly Walkthrough, Pre-Trip Inspection, etc.)
  • Responsible for Knowledge Company Policies and Procedures and Company Forms
  • Audit and Serve as the Custodian for all Compliance and Facility Checks

Qualifications

Skills/ Special Requirements:

• Must possess current California driver’s license or Identification Card.

• Must possess or obtain CPR and First Aid Certifications in allotted timeframe.

• Must have sensitivity to and ability to embrace individuals of different genders, age, sexual orientation, ethnicity, religion, physical abilities and those with diverse cultural backgrounds.

• Must possess a current RADT-1 Certification or similar credential or receive one in the allotted timeframe.

• Attendance in appropriate support group is required, as directed by supervisor, in order to gain a better perception of the disease they will be working around or with on a daily basis in order to model 12 step principles. (AA, NA, OA, Al-Anon, etc.)

• Possess the ability to make independent decisions when circumstance warrants such action.

• Possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.

• TB test to be conducted yearly or be verified by a physician.
