The Resident Care Coordinator will utilize the concepts of wellness to promote residents’ independence and delay decline. He or she will collect data on residents’ needs, coordinate and monitor staff and provide and reinforce the necessary training to meet those needs. He or she will assist in the oversight of systems and medical records within the community, especially the medication management systems.

This individual works closely with all aspects of the operation, including, but not limited to the: dietary, maintenance, activities, housekeeping, and resident care departments. He or she must be able to fill in for the manager at any time. The Resident Care Coordinator primarily oversees the medical records, medication management systems, and staffing. The Resident Care Coordinator must be able to be assigned by the facility R.N. to pass medications and be delegated to perform treatments and medical procedures including, but not limited to subcutaneous injections (insulin shots). Additionally, the Resident Care Coordinator must be well versed in all operational policies, procedures and reporting systems. This individual must have a detailed knowledge of the values and expectations of the company and must be able to communicate this information to staff in an effective manner. He/she must work to maintain a clean, safe, and comfortable environment that meets the needs of all residents.

Essential Duties And Responsibilities
The Resident Care Coordinator must:

Meet the legal and corporate qualifications to manage a residential care facility.

  • Have at least 1,040 hours (six months) of work experience in a residential care environment.
  • Fill-in in a variety of positions in the event of staff illness, or absence.
  • Demonstrate high standards of professional and ethical conduct.
  • Be alert, accurate and have sufficient dexterity to handle facility equipment safely and efficiently. Able to work under stress and, stand, walk, lift, and bend and stoop most of the day.
  • Be able to secure the cooperation of others, and be willing to cooperate with other personnel. Ability to lead and take direction.
  • A genuine interest in geriatric work, and interests of the facility.
  • Be on call to support staff and staffing requirements.
  • Be able to be assigned to pass medications.
  • Be able to be delegated to perform treatments and procedures.
  • Be knowledgeable with the computer software that the facility uses.
  • Have sufficient communication skills and knowledge of day-to-day operations of a residential care facility to orient staff as required by state rules and regulations.
  • Be able to manage medical records documentation systems, including, but not limited to: medical administration records, transcribing doctors’ orders, resident history files, activities of daily living documentation, and facility communication systems.
  • Be able to perform resident assessments.
  • Be able to perform routine audits, on records and duties, to ensure that services are communicated, delivered, and documented as prescribed by resident assessments, physician orders, and rules and regulations.
  • Develop staffing schedules to support the needs of the residents.
  • Maintain assignment (task) sheets for care staff, bathing schedules, and monitor daily to ensure compliance.
  • Monitor facility and legal safety requirements.
  • Ability to assist with the modification of resident behavior in accordance with the care plan monitor staff interventions, and recommend changes.
  • Other duties as assigned.

EDUCATION And/Or EXPERIENCE
Two years of college, from an accredited institution, preferably with an Associates Degree and six months experience working in a residential care environment, OR a high school diploma or general education degree (GED) and one to three years related experience and/or training.

Plus:

  • Experience in senior housing, assisted living or residential care and knowledge of medication systems and procedures, OR
  • Experience in geriatrics.
  • Experience with Point Click Care

Language Skills
An Resident Care Coordinator shall have sufficient communication and language skills to enable them to perform their duties and interact effectively with residents and other staff. He of she must be able to read, write, and comprehend instructions, business correspondence, and memos. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization

Legal Requirements
Employees in this position must successfully pass all governmental and corporate required background investigations, such as, but not limited to, state required criminal history clearance investigations.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: 30 – 40 per week
