The RN is a member of the care team responsible for all day-to-day coordination and management of care needs and care plans for our local office in Medford, OR. We are looking for a proven leader who’s committed to building the future of in-home care by delivering a best-in-class experience for clients, caregivers, and our staff.
You will be responsible for thinking through the client journey and how we can continue to optimize the client onboarding experience from inquiry to assessment to client care and connection. Additionally, you and the team are responsible for the smooth coordination of operations to achieve quality agency standards and positive financial outcomes.

Responsibilities:
Facilitate client access to community resources including locating recreational activities, housing, food, clothing, and relevant mental health services.
Maintain comprehensive client files by documenting all client encounters.
Complete and submit documentation to the accounting office as requested.
Identify and provide emergency crisis services as necessary making immediate clinical assessments by responding according to accepted methods and techniques.
Perform basic hands-on tasks of nursing such as urethral catheterizations, injections, etc. as required.
Appropriately counsel field employees on the expectations of their job utilizing acceptable coaching and disciplinary action as necessary.
Receive and process telephone calls from caregivers, clients, family members, hospital staff and other medical professional offices.
Qualifications:
Current RN licensure in the State of Oregon in good standing
Prefer a minimum of 5 years experience in direct patient care (triage, assessment and clinical skills).
Reliable transportation and current Oregon Drivers License with auto insurance.
Effective knowledge of community based resources and services for the elderly.
Creative problem solving skills with ability to manage challenging situations.
Excellent time management and organizational abilities with strong team player attitude and team building skills.
Ability to handle multiple tasks simultaneously while determining priorities and managing a changing environment.
Ability to monitor and assess physical status of clients and plan, implement, and evaluate individual patient care programs.
Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community and office workplace.
Ability to gather data and complex information while making autonomous decisions using best practice standards of nursing.
Ability to communicate medical information to health care professionals and the public over the telephone under normal and crisis circumstances.
Knowledge of the development of health care and client contracts preferred.
Ability to work independently and be self directed.
Must be conversant in the Oregon State Board of Nursing scope of practices and procedure.
Satisfy drug screening and criminal background screening.
