The Day Support DSP Team Lead-A is a support position to the Day Support Manager and New Point Life Services. The position leads, inspires, motivates, and provides suggestions to help the day support program and team reach goals in the workplace. The Team Lead will demonstrate and provide the most compassionate, least restrictive, community-based care for the individuals to ensure the highest possible quality of life. This Team Lead is responsible for components of Annual ISP planning, ISP revision planning, Person-Centered Quarterly Reviews, progress note compliance, behavior and serious incident reporting compliance, skill building activities, and overall implementation of person-centered practices.

Duties And Responsibilities

  • Perform DSP job duties.
  • Facilitate and oversee skill building activities weekly according to the goals outlined in the individual's ISP plans.
  • Facilitate engagement within the Day Support program daily during designated time frames.
  • Initiate and facilitate the completion of daily documentation within the Day Support program during designated time frames.
  • Check progress notes daily on a percentage of individuals to ensure that program-based activities, skill building activities, and community activities & outings are documented properly.
  • Update the progress note tracking spreadsheet located in the email drive weekly.
  • Ensure completion of GER reports following serious incidents occurring in the provision of Day Support services is completed and accurate before the end of the shift.
  • Ensure completion of Behavior Data following behavioral incidents occurring in the provision of Day Support services is accurate and completed before the end of the shift.
  • Facilitate and oversee completion of behavior data on forms outside of Theraps (interval data, ABC sheets, etc.).
  • Complete Documentation Observation forms weekly on progress notes and provide feedback to staff for improvement.
  • Complete Behavior Data Observation forms weekly on behavior data entries and provide feedback to staff for improvement.
  • Conduct 1:1 documentation training for new hires and current employees when needed.
  • Facilitate, participate, and contribute to monthly staff and management meetings as it pertains to quality documentation and person-centered plans.
  • Participate in annual ISP meetings for individuals who attend the Day Support program.
  • Communicate with the Records and Planning Department regarding ISP Plan Revisions when they are needed.
  • Write ISP Part Vs for the Day Support program and submit to the Planning and Records Department.
  • Write Person-Centered Quarterly Reviews monthly, ensuring that the information covered in each review is complete and accurate.
  • Meet documentation submission deadlines for ISP plans and Person-Centered Quarterly Reviews.
  • Participate in Intake and Discharge meetings for individuals attending the Day Support program.
  • Greet visitors and Support Coordinators and ensure proper sign in and field questions.
  • Correspond with case managers, family members, and potential individuals in a professional manner.
  • Participate in scheduled staff meetings, required trainings, and required certifications.
  • Work collaboratively with the manager to ensure the DSP's implement support instructions according to the individual's plan.
  • Act as support to Manager when on vacation or off-site.
  • All other duties as assigned.
