Job Description

Join us to bring positive change to the community! Seeking case managers (QMHP or QMHP-T) to provide services as part of our Dialectical Behavior Therapy team. The New River Valley Community Services DBT Team delivers evidenced based care inclusive of therapy, case management, and mental health skill building. NRVCS supports DBT case managers with regular professional consultation and agency and team structure supports easy coordination amongst other services on behalf of your client. The DBT team offers innovative, supportive services for our clientele (to aid in the practice of the skills within the community). We aid often underserved individuals, and teach skills for emotion regulation, distress tolerance, mindfulness, and interpersonal interactions to aid clients in achieving their “Life worth Living” goals.

Transitional Care Case Manager II:

Starting Salary: $35,984--Depending on Experience

Transitional Care Case Manager III:

Starting Salary: $37,793.60--Depending on Experience

Qualifications

  • Requires demonstrated knowledge of case management/social work practices, and resources available to help children and adolescents with serious mental illnesses, substance use disorders and individuals with co-existing disorders.
  • Working knowledge of DSM-V, etiology, symptoms, treatments, prognoses. Skills in assessment and service plan development is helpful.
  • Knowledge and understanding of best practice treatment modalities for serious mental illness, intellectual and developmental, substance use and co-occurring disorders to include behavioral supports and interventions.
  • Basic knowledge of human rights, ethical, and legal concerns as they relate to providing case management clinical services.
  • Working knowledge of Medicaid guidelines as they pertain to State Plan, Waiver, and Clinic Option Medicaid.
  • Must be able to demonstrate problem-solving, critical-thinking ability. Should be able to recognize early escalation of symptoms and/or situations and respond appropriately with de-escalation techniques.
  • Must be able to exhibit initiative, resourcefulness, and the ability to work independently and as part of a team.
  • Demonstrated ability to communicate clearly and concisely (written and oral), working knowledge of computers/word processing.
  • Must possess a valid driver’s license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment.
  • Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment;
  • Must have acceptable drug screen results upon hire.

Education/Experience Required:

Transitional Care Case Manager II:

  • Bachelor's Degree in education, social work, psychology or human services related field.

Transitional Care Case Manager III:

  • Bachelor's Degree in education, social work, psychology or human services related field.
  • Registration as a QMHP or trainee is required. Must maintain QMHP status and all required trainings/CEUs.

Additional compensation will be considered for any new employees who speak multiple languages.
