Responsibilities:
- Assist patients in understanding their diagnosis, treatment options, and the resources available.
- Assess learning needs, provide education then re-assess the learning achieved while providing support and navigation to patients with cancer and their families.
- Serve as an essential link between patients and all other care providers.
- Facilitate appointments for consults and support services within established service standards, including case managers, dietitians, physicians, & other services as needed.
- Promote communication between the patient and health care providers.
- Develop patient education programs and tools.
- Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
- Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes.
- Deliver quarterly report to Southeastern Med committees and other groups which demonstrate outcomes and performance improvement activities.
- Coordinate outreach efforts to establish and maintain positive working relationships with key customers, including but not limited to physicians, office staff, diagnostic staff, nurses, radiology staff, social service staff, and radiation/medical oncology staff.
- Participates in quality control and assurance.
- Participates in process improvement activities.
- Will participate in some aspects of clinical care including nursing services at the cancer center as needed.
- Perform other duties within the areas of Cancer Center as may be necessary from time to time, which are within qualifications and as directed by supervisor.
Qualifications:
Associate's or Bachelor’s degree in nursing.
Licensed and currently registered(or eligible) as a professional nurse in the State of Ohio.
3-5 years nursing experience in oncology.
Patient navigation certifications related to oncology services preferred.
The Oncology Services Patient Navigator must successfully complete a recognized patient navigator-training program or provide documentation of the requisite knowledge and skills from previous education and experience to provide patient navigation within one (1) year of hire date.
Associate's or Bachelor’s degree in nursing.
Licensed and currently registered(or eligible) as a professional nurse in the State of Ohio.
3-5 years nursing experience in oncology.

Position reports to Administration/CNO.
The position is day shift, M-F from 8am-4:30pm.
The hospital offers an hourly rate 63K-73K depending on experience and they offer a full benefit package.
