This is a full-time day position Monday thru Friday 8:00-4:30pm
Job Summary:
The COVID-19 Care Team CHW uses their expertise about the community they are working with to provide education about COVID-19 and chronic disease prevention and treatment.
Essential Functions:
Engage in community outreach and education about chronic disease screening and health
Learn and apply clinic policies and procedures to Care teamwork
Educate community about chronic disease prevention and management
Engage in relationship-building discussion with patients
Log enrollments, educational activities, and referrals
Track and follow-up with patients as part of clinical workflows, supporting team-based care
Position Qualifications:
Have the ability to work independently with minimal supervision
Possess the ability to encourage others to seek opportunities for different and innovative approaches to addressing problems and opportunities
Great written and oral communication skills and be detail oriented
Must have current driver
