The Case Manager RN plans, organizes and directs the provision of Social Work and Case Management. You coordinate with nursing, ancillary departments, patients and their families as well as appropriate community agencies to assure appropriate stay and discharge is obtained.
Essential Functions:
Must possess good assessment and interpersonal skills to formulate an appropriate discharge plan, direct the patient and/or family to appropriate resources and to provide crises intervention
Must possess ability to speak to groups and provide leadership for support groups and rehabilitation programs
Regularly demonstrates the ability to exercise judgment and decision-making capabilities to ensure optimum level of patient care services
Is knowledgeable about community resources, financial and social systems and medical terminology
