Care Management Coordinator - ACN - Pediatric Complex Care Management

As a Care Manager within our ACN (Ambulatory Care Network), you will be responsible for providing intensive care management for pediatric patients with complex medical needs. Your practice will involve developing individualized care plans, and collaborating with an interdisciplinary team of physicians, advanced practitioners, social workers and other stakeholders, to provide a comprehensive approach to care. Help minimize and prevent hospital readmissions throughout our health system for patients of special needs. Guide patients and caregivers as they navigate the complex world of primary and specialty care, screenings, treatment, and recovery. Follow up with patients and their families to ensure their understanding of the plan of care, resources within their community, access to care, medication management and home care needs. Utilize your nursing expertise to serve each patient’s unique needs, while ensuring quality and continuity of care. Form strong and enduring bonds with patients, families and multidisciplinary colleagues.
This is a fulltime position, Monday-Friday 9:00am-5PM. The position is hybrid, 3 days onsite and two days remote.
Preferred Criteria
Two years case management in an inpatient, HMO, Insurance or other outpatient/ambulatory setting
Master's Degree in healthcare related field
Required Criteria
Bachelors degree in Nursing (BSN)
Current NYS Registered Nurse (RN) licensure
3 to 5 years of health care experience
Flexibility and ability to work in fast-paced, changing environment
