Description
Under the direct supervision of the nurse, the Certified Nursing Assistant assists in the direct patient care and assists patients with activities of daily living.
JOB SPECIFIC RESPONSIBILITIES:
Completes and records routine patient vital signs as per instructed by RN/LVN.
Performs patient rounds at least every 2hrs, if unable to perform, primary nurse is notified.
Knowledgeable of vital sign ranges and reports any deviation to the primary nurse immediately.
Ensures fluid/food intake and output are appropriately measured and recorded in patients medical record every shift.
Bathes patients as scheduled, if a patient declines, nurse is properly notified and document in patients medical record.
Performs oral care at least once per shift and documents in patients medical record.
Provides hair care at least once per shift and documents in patients medical record.
Ensures patients are turned at least q2hrs, if patient cannot be turned or refuses, the primary nurse is notified and is documented in the patients medical record.
Feeds patients per restrictions as assigned/required.
Understands dysphagia and appropriate diet instructions.
Assist with safe transfers as needed.
Assists patients with bowel/bladder needs.
Answers patients call lights-timely (within 5 minutes if not in another patients room). Immediately reports any patient concerns/patient family complaints to primary care nurse.
Reports any perceived/actual change in patients alertness, physical condition, skin changes, and any other areas of concern to patients primary care nurse immediately.
Able to verbalize hospitals emergency codes and responsibility with each.
Can verbalize/perform the CNA role in patient emergency situations and cardiopulmonary arrest.
Ensures patients bed is clean and organized at all times with call bell and patient items within reach.
Practices correct isolation techniques to maintain a safe environment and to control spread of infection.
Retrieves patients equipment from materials management as required/requested.
Maintains cleanliness of various instruments and equipment used per hospital policy and returns equipment to Central Supply. Reports non-functioning equipment to Central Supply.
Keeps patient rooms clean and in an orderly condition by removing all trash and preventing unnecessary clutter.
Assist with restocking rooms with supplies and equipment as required including gowns, gloves, masks, etc.
Communicates with patients, patients family, staff, and physicians in a professional and courteous manner.
Exchanges and communicates patient care information upon change of shift.
Performs walking rounds at change of shift.
Completes all required documentation in a timely manner and prior to end of shift.
Ability to accept direction from supervisors and other management team members in a positive and professional manner.
Returns all soiled trays upon patient meal completion and prior to scheduled dietary tray pick up times.
Ensures soiled linen hampers are covered, cleaned and/or emptied as needed and at the end of the shift.
Maintains clean materials/supplies separate from dirty materials/supplies.
Communicates unit/patient concerns to House Supervisor/Nurse Manager.
Performs and documents patients weights as ordered.
Performs bedside glucose checks, reports results to primary nurse, and documents values in the patients medical record.
Updates patient communication board daily for assigned patient group.
Interacts with patient and families in a caring and compassionate manner.
Participates in orientation of new CNAs.
Attends at least 80% of all staff meetings.
Attends at least 80% of all education at Nexus Specialty Hospital.
Participates as a team member of the Quality Management System.
Performs all other duties as assigned.
Job Requirements
Experience
Prefer at least six (6) months experience working in a hospital/rehabilitation/long-term care environment
Education Required
High school Degree or equivalent
Additional Information
LICENSURE/CERTIFICATION:
- Certified as a Certified Nursing Assistant in the state of Texas
- Certified in CPR
