Duties and Responsibilities:
- Provide primary care consistent with CCMD mission of bringing dignity to fragility and
- aligning care so that patients can live their best lives.
- Provides care for patients in skilled nursing ,long term care, ALF , B & C and at home.
- Manages a complex caseload of patients. The performance standard is at least 260
- required visits per month.
- Documents thoroughly and accurately in the CCMD EMR.
- Completes chart notes, to include visits and discharge summaries in accordance with
- CCMD policies.
- Reviews and orders medications (including IVs), labs, and other diagnostic testing.
- Participates in patient’s Interdisciplinary Team Conferences.
- Available from 7 AM – 7 PM for phone calls from Monday – Friday.
- Participates in rotational after hours on-call which may include weekends and holidays,
- (reimbursement as per company policy), after hours call from 7 PM – 7 AM.
- Participates in QI program, Peer Review and Utilization management meetings.
- Conducts advanced care planning discussions with patient and family to include POLST
- completion.

- Maintains ongoing communication with patient and family on goals of care, change in
- condition, and other issues impacting the patient’s health and emotional wellbeing.
- Proactively communicates with case manager, primary care physicians, specialists and
- other members of the care team.
- Attends and participates in monthly staff meetings.
- Assesses patients to identify needs for services and support.
- Communicates assessment outcomes with patients, PCP, and key service providers.
- Serves as patient advocate and facilitator to resolve issues that may be perceived as
- barriers to care.
- Performs other duties and responsibilities as assigned.
- Strong background in geriatric and/or internal medicine
- Demonstrate ability to work independently
- Thrives in an unstructured, start-up environment.
- Excellent communication and interpersonal skills with the ability to effectively
- communicate with all levels of management, patients, and family members.
- Creative, flexible, well organized, resourceful, and detail-oriented
- Excellent judgment in handling confidential and sensitive information
- Establishes and maintains cooperative working relationships with others
- Ability to work across locations and time zones
- Occasionally require lifting or exert force up to 10 pounds.
Qualifications:
- Master’s degree with an emphasis in adult/geriatric program
- Must be Board Certified.
- State certification as adult/geriatric nurse practitioner / PA
- Current CPR certification
- DEA
- Experience in value-based care
