The Nurse Family Partnership (NFP) Nurse Home Educator (NHE) functions in a multifaceted role that demonstrates professionalism and promotes excellence in the practice of Nursing. NFP is a home visiting program from pregnancy to the child’s second birthday. The NHE is responsible for delivery of comprehensive community health nursing care to first-time, low-income pregnant women. The Nurse Home Educator maintains high standards in clinical practice/continuum of care, professional practice development, teamwork, customer relations, and research/performance improvement in delivering services for a designated caseload of patients. NHE implements and maintains the fidelity to the NFP model. Our goal is that all children are healthy, families thrive, can the cycle of poverty is broken. The nurse be able to work independently and must have a valid driver’s license.
