In this rewarding role, you'll work collaboratively with physicians and other healthcare professionals to ensure the health and well-being of our residents, focusing on preventive care, chronic disease management, and addressing acute needs.
Responsibilities :
- Conduct comprehensive physical examinations, diagnose and treat acute and chronic illnesses in a geriatric population.
- Develop and implement individualized care plans that address residents' medical, physical, and psychological needs, in collaboration with the supervising physician.
- Order and interpret diagnostic tests, prescribe medications, and perform procedures within your scope of practice as authorized by the supervising physician.
- Collaborate with physicians, nurses, social workers, and therapists to provide coordinated and comprehensive care.
- Educate residents and families about health conditions, treatment options, and preventative measures.
- Advocate for residents' well-being and ensure they receive appropriate care.
- Document patient care accurately and thoroughly in the electronic health record (EHR).
Qualifications :
- Active Physician Assistant (PA) license in good standing.
- Minimum 2 years of clinical experience in a geriatric setting (nursing home, long-term care, or geriatric clinic) preferred.
- Strong clinical skills in geriatric assessment, diagnosis, and treatment.
- Excellent communication, collaboration, and interpersonal skills.
- Compassionate and patient-centered approach to care.
- Proficient in EHR documentation.
- Must be able to work effectively under the supervision of a physician.
