Overview
The Community Support Services Supervisor (CSS) is responsible for the ground level operations in one of our Community Support Services, which is located in Virginia and Chisholm. The CSS provides leadership and supervision to the community support staff working in the assigned program, helps manage program and consumer finances, generates staff schedules, train in new staff, works with the Director of Community Supports to develop and implement consumer goals, ensures a person-centered environment, and leads their team in an effective and positive manner to ensure consumer plans and goals are followed through.

Responsibilities

Main contact for case managers, guardians, family and other client-related stakeholders and represents the organization as directed by the Director of Community Support Services
Supports, cooperates with, and implements specific procedures and programs for:
Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management and security plans
Reports and/or corrects unsafe working conditions
Confidentiality of all data, including client, employee and operational data
Quality Assurance and compliance with all regulatory requirements
Supports and participates in common teamwork:
Cooperates and works together with all co-workers; plans and completes job duties with minimal supervisory direction, including using appropriate judgment
Uses tactful, appropriate communications in sensitive and emotional situations
Follows up as appropriate with supervisor, co-workers or clients regarding problems and concerns
Promotes positive public relations with clients, family members and guests
Completes requirements for in-service training, acceptable attendance, dress codes (including personal hygiene)
Coordinate effectively with external partners such as case managers, guardians, family members and referral sources to promote client retention and acquisition of new contracts
Review employee and client documentation and provide feedback to staff as appropriate
Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior
Ability to have a flexible schedule and availability to meet the needs of the program, clients and employees as determined by agency leadership and works direct care as needed to support the programs
Observe and provide employees with consistent feedback and support to ensure development and the adherence to company policies/procedures; provide instruction, respond to concerns/issues; and provide support as needed
Address employee discipline within a timely manner as determined by Human Resources and/or Executive Team
Assure, supervise, provide, and maintain quality care to the individuals we serve in an environment which promotes their rights, dignity, freedom of choice, independence, and their individuality
Assist the immediate supervisor with various administrative tasks to support the worksite operations as directed; may be assigned special projects
Is responsible for all aspects of program specific training for new employees; ensure employees attend scheduled trainings and events as required by the agency
Create & monitor employee schedules in accordance with program needs and within designated budget:
Supervised staff will have advanced notice of their schedules
Review and approve time off and overtime
Communicate and cooperate with all agency departments
Arrange and facilitate staff meetings
Meet on-going requirements to maintain required certifications and participate in professional training and development as assigned
Is knowledgeable of client rights and ensures an atmosphere which allows for privacy, dignity, and well-being of all individuals we serve in a safe, secure environment
Develop and adhere to and oversee clients’ Self-Management Assessment Plan & Individual Abuse Prevention Plans, Coordinated Service Support Plan Addendums, and the Person Centered Planning approach in identifying and meeting service needs:
Facilitate client’s outcomes and daily service delivery
Instruct and assist support staff with implementation
Evaluate outcomes and delivery
Have an understanding of the diagnosis, medical, mental health and behavioral needs of the clients
Write and coordinate progress reports as defined by the interdisciplinary team (quarterly, semi-annual and/or annual reports)
Responsible to gather, monitor and disseminate information and data related to outcomes, medical appointments, and psychotropic medications and delegate to a team member as necessary
Oversee all client appointments and delegate to a designated team member as deemed necessary
Maintain and manage budgets, as applicable, by reviewing reports generated by Community Support Services Direct Care staff, Community Support Services Director, and/or Executive Team to ensure compliance with annual budget

Qualifications
Experience/Education/Training
High School Diploma
Meet the qualifications of a “Designated Coordinator” as identified in Rule 245D as follows:
Bachelor’s degree in a field related to human services AND one-year full time work experience providing direct care to persons with disabilities and/or persons 65 or older
Associate Degree in a field related to human services AND two years of full-time AND one-year full time work experience providing direct care to persons with disabilities and/or persons 65 or older
Diploma in a field related to human services from an accredited post-secondary institution AND three years of full-time work AND one-year full time work experience providing direct care to persons with disabilities and/or persons 65 or older
Minimum of 50 hours of education and training related to human services AND four years of full-time work AND one-year full time work experience providing direct care to persons with disabilities and/or persons 65 or older
At least two (2) years of experience supervising others working with individuals with developmental disabilities is required which must include significant experience:
Providing support for consumers during times of change, such as entering or leaving programs or end-of-life care AND
Experience coordinating with external professionals AND
Working in multi-need programs AND
Providing training, support and direct supervision for staff
