Licensed Practical Nurse (LPN) Supervisor Job Description – Private Duty

No Place Like Home Health is a small Veteran owned home care agency that is looking to add a LPN Supervisor to our team. For this position, you will complete supervisory visits at client's homes as well as perform staff management for our Non-Medical staff which includes Home Health Aides and Certified Nursing Assistants. You must be willing to commute to our clients' homes as well as the main office in Livonia. This position is split half out in the field and half in the office.

Functional Role and Status

· The LPN Supervisor is responsible for overseeing and educating subordinate staff, completing patient assessments, medication administration, as well as acting as the Clinical liaison for patients and their families.

Requirements and Qualifications

  • Education: Graduate from an accredited school of Practical Nursing.
  • Licensure: Must be licensed in the State of Michigan as a Licensed Practical Nurse.
  • Experience: Must have a minimum of two (2) years of community health or private duty nursing experience.
  • Must have a minimum of one (1) year of nursing experience in a facility or hospital setting.
  • Must have at least (1) year experience of management.
  • Must have home health scheduling experience
  • Clear Care experience preferred
  • Covid-19 Vaccination Required
  • Must be proficient in Microsoft office and computer programs
  • Must have customer service experience
  • QAPI experience preferred

· Skills, Knowledge and Abilities:

o Working knowledge and demonstrable skill in licensed practical nursing.

o Must have excellent verbal and written communication skills, interpersonal skills, and sound professional persona with professional outward appearance.

o Ability to identify and analyze potential problems or inefficient processes and come up with solutions to solve and/or prevent them.

o Ability to work independently, be organized with attention to detail and have the ability to make sound clinical/nursing decisions.

o Willingness to understand, support and put into practice the philosophy, core values, objectives, policies, and procedures of the company.

· Physical: Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patient care. Physical activities will include walking, sitting, bending, and standing; in addition to lifting, repositioning, and transferring of patients.

· Mental: Requires emotional stability and composure in stressful situations.

· Availability: Flexible scheduling, to include day, afternoon, and evening hours. Occasional weekends and holidays may be required.

· Working Conditions: Temperature controlled office environment with an assigned workspace, including frequent exposure to noise and a regular flow of people around the office. Also, within a community setting, including facility and private resident apartment and homes.

· Required Checks: Criminal history check, professional licensure verification and professional references.

· Compliance: All personnel are required to provide and keep up-to-date records with the HR department. Professional license(s) and/or certifications should also be provided and updated as necessary. All compliance documentation should be provided and recorded within the HR department.

Reporting Relationship

The LPN Supervisor reports directly to the Owner/Administrator but must accept direction from other Directors and work collaboratively with all personnel within the company.

Authority

· Authority to put into practice, inform and hold subordinate personnel accountable for all organization policies, procedures, practices, and written guidelines as they relate to the position responsibilities and duties.

· Authority and responsibility for maintaining organization compliance and clinical standards for all subordinate staff.

Responsibilities and Duties

· Provide nursing care and perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care.

· Make decisions that are based on nursing education and experience.

· Assist with the preparation, implementation, and continuing evaluation of patient care plans.

· Educate and teach patients the appropriate self-care techniques.

· Complete patient assessments as scheduled or as needed.

· Observe, record and report to the appropriate person patient symptoms, reactions and changes including:

o The general physical and mental condition of patients and signs and symptoms which may be indicative of changes in condition.

o Stresses in relationships between the patients and staff and between patients and their families or visitors.

· Prepare and set-up patient medications on a daily, weekly and/or monthly basis per physician order; communicate as necessary with the involved physician and pharmacy to resolve any issues or errors.

· Educate and train staff on medication delivery and documentation.

· Supervise staff both in a facility and home-based setting, complete supervisory visits according to schedule and per company policy and document said findings.

· Educate, train and coach staff on proper patient care procedures. Offer performance feedback, including areas of opportunity or those needing improvement. Communicate all concerns to the HR department in a timely manner.

· Assists with facilitating New Employee Orientation and other staff in-services and training.

· Performs other duties as assigned and contributes to the team by accomplishing related results as needed.

Job Type: Full-time

Pay: $40,000.00 - $47,000.00 per year
