What are the responsibilities and job description for the Behavioral Health Navigator position at North Alabama Shoals Hospital?
Our acute-care psychiatric hospital provides stabilizing care for adults, and seniors, who are experiencing a mental health crisis or behavioral health conditions or dementia.
Continual assessment of patient needs, and development of individualized care plan with measurable goals to ensure needs are met. Goals include both physical health and behavioral health.
Assist patients with follow up appointments when discharged from the hospital.
Ensure patients daily psychiatric, medical, and environmental needs are planned for and coordinated.
Meet regularly with Care Management Supervisor (individual and group).
Attend morning and weekly team meetings as scheduled. Follow-up with the plan(s) developed in each meeting.
Facilitate education of consumer and/or involved collaterals to psychiatric diagnosis and appropriate treatment options.
Develop and maintain relationships within family unit and friends.
Ensure timely and appropriate documentation as required.
Develop and maintain community relations through prompt follow through with contacts.
Develop and maintain relationships within family unit to support the immediate and/or extended family.
Excellent communication skills.
Demonstrates initiative in problem solving.
Must possess the ability to conduct initial assessments and clearly communicate the findings to the healthcare providers.
Must collaborate with the multidisciplinary team to ensure BH patients receive quality care.
Provides education to patients and families about the patient’s conditions, treatments, medications, and side effects.
Assists with admissions and discharges to ensure continuity of care. Assist with making appointments for follow up care with local behavioral health resources.
Serves as a liaison between patients, families, and physicians.
Assist in translating medical jargon into information the patient and family can understand.
Promote and demonstrate professional standards to enhance the development of the Behavioral Health program.
Assist with other nursing duties as requested by the nurse manager or director of the Behavioral Health program.
