PART TIME ROLE
PAY: $32hr
Schedule: Part time - 3 days a week, 8AM to 5PM (24 hours)
Location: Coverage for both Norwalk and Long Beach PACE locations

Job Summary for MSW Social Worker:
Responsible for participants assigned to the team and works closely with the provider and nurse to coordinate services and advocate for
participants. Responsibilities include, but are not limited to conducting family conferences, assisting with community referrals, regular
contacts with participants, and incident reporting
Essential Job Functions:
1. Responsible for quarterly notes and developing care plans for assigned team/participants of Senior Services Program(s) and
complete within monthly timeframe set by state guidelines.
2. Interview and screen SCS participant’s family members and caregivers to indentify additional need for psychotherapeutic
intervention and inform immediate supervisor.
3. Provide information and referral to SCS participants and family and assist with coordination of services through public, private and
social services agencies.
4. Act as a liaison with participant’s family, caregiver and with other community agencies providing services to SCS participants.
5. Document all SCS participants and family contacts in accordance with SCS’ documentation guidelines.
6. Works with Care Coordination/Management Team (RN, OT, PT, Activities) by taking lead in investigating non-clinical psycho-social
problems as identified by the multidisciplinary team and document in NextGen and/or appropriate program electronic systems.
Qualifications
1. Masters degree in Social Work from an accredited university required.
2. Two years experience in case management/social advocacy with geriatric population required.
3. CPR/First Aid certificate required.
4. Bilingual English/Spanish, English/Tagalog or another second language strongly preferred.
