The Care Coordinator is an integral part of the treatment team for youth and adults who receive services from Acute Care Services – either the crisis stabilization programs or inpatient behavioral health hospitals. The Care Coordinator provides support to clients’ individual treatment plans while in the facility and provides the client with recommendations and referrals upon discharge such as to social services, community supports and resources, and placements in outside care systems. The position is responsible for coordination of care with family members and other providers regarding the needs of the client.

Crisis Stabilization Facility - Crisis stabilization programs provide short term voluntary therapeutic interventions to reduce and stabilize a person’s mental health crisis and link them to other resources and support

Behavioral Health Hospital - The hospitals provide short term voluntary and involuntary 24/7 inpatient psychiatric care and related services to youth ages 13-17, and to adults.

Education, Licenses, Certification, Skills and Experience Requirements:


  • Minimum Education: Bachelor’s degree in social work or related field earned from an accredited college or university academic program

  • Minimum Work Experience: One to three years’ human services program related work experience and one year related to discharge planning or resource coordination for similar populations.

  • Required Skills: Experience working with people with mental health diagnoses

  • Driving Requirements:
    • Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care.
    • Must have the use of a personal vehicle.

  • Preferred Qualifications, Skills, Education, Licenses, Certifications and Requirements:
    • Social work experience
    • Current Wisconsin Certified Social Worker
    • Current Wisconsin Substance Abuse Counselor or Substance Abuse Counselor In-Training certification

Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.

Essential Job Duties and Responsibilities:

  • Provides guidance to unit paraprofessional staff (Behavioral Health Professionals) relative to provision of services in treatment plans
  • Responsible for timely completion of psychosocial assessments, Safety and Crisis Plans, and recommendations included in Discharge Summaries.
  • Responsible for appropriate care coordination/Discharge Planning based on client need.
  • Educates parents/guardians of discharging youth/clients on successful strategies used in treatment plan
  • Makes appropriate and timely referrals to other agencies based on client need and services required.
  • Provides support for individuals, families, and groups.
  • Assists in the completion of patient aftercare forms and serves as a client advocate.
  • Provides clinical review/discharge reports to insurance companies as appropriate.
  • Makes resource and support referrals to individuals utilizing crisis stabilization services.
  • Assures that client care provided in the department is completed in a respectful manner that upholds the dignity and rights for adolescent and adult clients.
  • Performs social counseling services for individuals, families, and groups.
  • Maintains regular on-site attendance

Non-Essential Job Duties:

  • Provides cross-coverage in other Behavioral Health Service programs as-needed to support the Department as a whole.
