Family Specialist – DULCE (Developmental Understanding & Legal Consideration for Everyone) - To promote the healthy development of infants from birth to six months of age and provide support to their parents, while proactively addressing the social determinants of health. This innovative intervention is through the pediatric primary care clinical sites. The Family Specialist is part of the pediatric interdisciplinary team.
- Serves as a catalyst in bringing the family into a closer relationship with their pediatric provider.
- Works in tandem with the pediatric medical team during all routine well-child visits (birth to six months) to strengthen their knowledge of their baby’s development; scaffold their emotional and relational capacity for mutual-regulation with their child; support parental strengths and process of mastery; and identify parenting strategies.
- Collaborates with Medical-Legal Partnership to ensure families are provided concrete supports in vulnerable situations
- Administers relevant screening instruments to best understand the child’s development and the process by which children develop. This includes understanding and anticipating developmental and relational “Touchpoints”- known periods of developmental and behavioral disorganization that occur before a new skill emerges that increase stress and potential risk in the family system.
- Provides ongoing parent/newborn support through home visits, telephone, email and community visits with a focus on parental and cultural strengths that can be brought to bear on the parenting and coping process.
- Continually assesses the need for extra resources for the family, especially related to developmental concerns, by building strong, honest, trusting partnerships with families that can address concerns in a supportive manner.
- Maintains regular communication with the Patient-Centered Medical Home team (primary care clinicians, social work, nursing and support staff) to share relevant information on patient care.
- Participates in weekly case conferences with DULCE Clinical Team.
- Participates in monthly team/CQI meetings.
- Participates in DULCE technical assistance and quality improvement activities.
- Participates in outreach and community events.
- Participates in related trainings and workshops.
- Completes all health record charting requirements.
- Participates in data collection for DULCE Registry and as assigned or requested.
- Performs other related duties as assigned or requested.
- Maintains and enhances job skills through the use of supervision and other professional development opportunities.
- Participates in required trainings, outreach activities, supervisions, and meetings as assigned.
- Measures and documents outcomes as outlined in contract.
- Complies with all agency policies and paperwork requirements.

Requirements
- Must comply with NEKCA policies if hired
- Required to authorize and pass NEKCA’s criminal record check process and DMV record check process
- Valid driver’s license, auto insurance and auto registration
- Bachelor’s Degree; or equivalent combination of related education, training and experience.
- Proficiency is required in a Windows Operating System to include competency in all applications in the Microsoft Office programs Suite. Other specialty software and applications abilities specific to this position may also be required. Additionally, proficiency is required with day-to-day information entry and retrieval in the Agency’s electronic record system(s).
- Adult/Child CPR, Basic First Aid, Handle with Care, Defensive Driving, Relias online trainings
- Ability to read, analyze and interpret a variety of general business documents, regulations, clinical documentation and reports including, but not limited to, legal and/or financial. Ability to write reports, business correspondence, clinical documentation and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, staff, and the general public.
- Ability to apply common sense understanding to carry out detailed written or oral instructions. Ability to problem solve with guidance or supervision. Ability to interpret instructions, solve practical problems and deal with a variety of variables where only limited standardization may exist. Ability to define problems, collect data, establish facts, and draw valid conclusions with little to no instruction.
