JOB DESCRIPTION:
An Activity Leader conducts recreational activities for high-functioning residents, whether it’s through activity groups or one-on-one activity therapy. Under the supervision of the Activity's director, this position also assists in planning, developing, coordinating, and implementing various levels of activities, amongst other daily operational tasks.
RESPONSIBILITIES:
- Assists in developing monthly activities schedule based upon individual and group needs, interests, and capabilities, accessible to residents and staff and can be easily read.
- Assists in preparing a written plan for the individual, group, and independent activities following their needs, interest, and capabilities.
- Documents all interactions with resident and family in the assessment, care plan, and progress notes as required by federal and state requirements.
- Notifies and assists the Activity Director in selecting safe and adequate space, equipment, and supplies needed to operate the activity's programs.
REQUIREMENTS:
- Minimum 17 years of age; high school diploma or equivalent preferred
- Self-motivated
- Excellent customer service skills
- Physically able to transport residents in wheelchairs
- Ability to read and understand work assignments, as well as report verbally and provide written documentation as necessary
- Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
