What are the responsibilities and job description for the Outcomes Manager- ER, FT, Days position at North Mississippi Medical Center?
Coordinates:
- Coordinates The Joint Commission (TJC) and Centers for Medicare/Medicaid Services (CMS) compliance activities of Core
Measures, Outcome Measures, and Value Based Purchasing metrics - Develops, implements, coordinates and evaluates the Performance Improvement (PI) programs/projects
- Plans, develops, and implements initiatives designed to meet TJC/CMS top performer status
- Utilizes scientific approach in the analysis of patient care issues, based on current, evidenced-based guidelines
- Identifies and tracks significant process and outcomes measures and compares to internal and external benchmarks
- Identifies quality care measure set guidelines and updates, facilitates the readiness of care providers through process changes and education, promoting optimal patient outcomes in a cost effective manner
- Assesses current PI processes and makes necessary recommendations
- Collaborates with patient care team to:
- Identify optimal processes and measures throughout continuum of care
- Move populations through continuum
- Develop structured care methodologies (order sets, algorithm, care paths)
- Evaluates complex patient progress along continuum, intervening and facilitating optimal patient outcomes
- Coordinates the assessment, planning, implementation and evaluation of educational programs intended to increase community awareness and promote positive patient outcomes (stroke care, cardiac care, etc.)
- Serves as Quality representative on various committees/teams (Performance Improvement, Policy and Procedure, Infection
- Control, TJC, etc.).
- Effectively meets all monitoring, evaluating and reporting of core measures through Premier data base and concurrent chart reviews
- Oversees the collection and analysis of data related to the outcomes management
- Effectively and efficiently communicate assessments, recommendations, resolutions as needed
- Adheres to NMHS/NMMC Policies/Procedures/Guidelines
- Complies with applicable Local/State/Federal policies/procedures/guidelines
- Bachelor’s Degree in Nursing or Health related field; required/ Master’s Degree; preferred
- Licensed as a Registered Nurse in the State of Mississippi; required
- Minimum of 5 years’ Nursing experience; required
- Certified by the American Heart Association (AHA) in CPR at the Health Care Provider (HCP) level; required.
- Knowledge of performance improvement, standards of care, regulatory agencies (CMS/TJC), and legal aspects of the nursing practice; required
- Quality data mining familiarity; preferred
- Excellent organizational and communication (written and verbal) skills; required
- Excellent analytical skills; required
- Excellent interpersonal skills; required
- Interaction with a patient population ranging from neonates to geriatrics; required
