Qualifications
Position Summary:
North Range Behavioral Health is a recipient of federal funding through the Substance Abuse and Mental Health Services Administration (SAMHSA) to be a Certified Community Behavioral Health Clinic (CCBHC). North Range Behavioral Health is now one of four centers in Colorado to join others across the nation. The CCBHC, or Access Team, is an innovative multidisciplinary team initiating immediate access to behavioral health care. The team includes clinicians, mental health counselors, and case managers working together to create a seamless and smooth front door for our large community behavioral health center in Weld County, Colorado.
As a Case Manager on the Access Team, you can expect to collaborate with clinical teams to secure and provide resources to clients with mild to severe mental health challenges and/or substance use disorders. Case managers work directly with clients across the lifespan to provide compassionate and comprehensive bridge care while clients await connection to ongoing services at North Range Behavioral Health, including education and skill development to help them move toward independence.
Duties Performed:
- Assist clients in achieving their goals as stated in their treatment plans or as indicated by appropriate brief interventions.
- Collaborate with therapists and other relevant staff to coordinate services, and address client needs and behaviors; prevent deterioration and/or hospitalization through consistent and adequate monitoring as indicated by program scope.
- Refer, coordinate, and assist clients to access a wide variety of services (e.g. vocational, medical, social security, housing, etc.) based on need and client desire; aid clients in application processes and provide supporting documentation as needed.
- Maintain close and cooperative relationships with community agencies, other public agencies, and private businesses (i.e. landlords, banks, etc.) in carrying out supportive services. May maintain contracts, leases, and other paperwork on behalf of clients.
- Facilitate client engagement in services and education. May teach approved curriculum for psychoeducational classes/groups with modification of teaching techniques to engage all participants in learning.
- Educate the client on behaviors to maintain daily life activities. Aid and guide clients with daily life activities (e.g. managing finances, medical treatment recommendations, physical and emotional self-care, etc.).
- Offer support, assistance, consultation, and education to relevant providers, family members/caregivers, and other involved parties when necessary to adequately support clients and minimize barriers.
- Ensure service engagement and delivery as necessary by adapting to client needs (i.e. visiting clients in various environments, conducting active outreach to clients, accompanying clients to various appointments where clinically indicated, setting up medications in med-minders, etc.).
- Assist clients in improving their well-being and quality of life by coaching in behavioral skills, educate clients to make informed choices; develop coping skills of clients; help clients to problem solve and negotiate tasks of daily living.
- Monitor, evaluate, and record client progress concerning treatment goals. Use electronic systems to document process reports, case management notes, letters, faxes, and phone calls. May input, organize, and maintain detailed clinical performance data to comply with applicable government program requirements.
Education/Credentials:
- A Bachelor’s Degree in Human Services field from an accredited college or university is required.
- A Valid Driver’s License and driving record that meets agency insurability requirements is required.
- Bilingual Skills (English/Spanish) desired
Experience:
- No experience is necessary but is preferred.

Salary placements will vary and may be above the starting wage based on factors including but not limited to location, experience, education, and licensure. Salary placements will generally occur between the starting wage and the midpoint of the salary range.
Employees may be eligible for additional pay based on program, evening, night, weekend, or holiday work. Bilingual pay may be available upon successful passing of language validation testing.
