Case Management work is done in communities settings, not in offices.
The Blended Case Manager works with the serious and persistently mentally ill population, using a strength-based perspective, to assist clients to obtain necessary services and support. The primary goal is to empower the client to identify their needs and choose the resources to fulfill them.
Requirements: Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education;
OR
A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years experience in direct contact with mental health consumers;
Must maintain a valid PA Driver's license.
