NCHC is looking for a Behavioral Health-Community Health Worker to join the team to provide services within our St Johnsbury Health Center. The Behavioral Health-CHW provides care coordination, community resource referrals, behavioral health brief intervention as well as mental health and substance misuse screening and assessment, including medication assisted treatment coordination. The opportunity involves working in the primary care settings to provide integrated behavioral health services as part of our patients’ care delivery team.
