Northern Light Home Care and Hospice is searching for an experienced and compassionate full-time hospice and complex care Nurse Practitioner or Physician Assistant . Here at Home Care and Hospice, we actively assist clients and families to achieve their highest level of personal health and independence. The ideal candidate would focus on clarification of patient and family goals of care, patient safety, appropriate coordination of care and patient self-management opportunities, all while delivering compassionate, professional, and high-quality care.
Position Highlights:
- Elevated level of schedule flexibility, with some weekends and holidays
- Collaborate directly with referring physicians and Medical Director on care plans
- Focus on clarification of patient and family goals of care, safety, coordination of care
- Assists clients and families to achieve highest levels of personal health and independence
