Case Manager - Six Stones Wellness Center (32-40 hours)

We are looking for a Case Manager to serve in the Six Stones Wellness Center. This position will work under the direction of the Program Manager and will offer non-crisis mental health peer to peer support in a Wellness Center setting and run peer groups. This center provides a member directed wellness program that fosters recovery and resiliency. These services included peer support, socialization activities as well as wellness and recovery activities. Any individual experiencing mental health challenges, and/or their family member(s), are welcome to participate in Six Stones activities.

Examples of Duties: Under the direction of the Program Manager and Program Supervisor, the Case Manager will provide and maintain a variety of case management services to clients. This may involve conducting individual assessments, creating service plans and quarterly reports, and providing ongoing training and intervention as outlined in each client’s individual assessment and service plan. Case management services are primarily provided at Six Stones Wellness Center and once a week to Mt. Shasta Wellness Services. Train and oversee interns assigned to the program. Compile monthly, quarterly, and year-end reports for the agency and funding agencies. Occasionally participate in recruitment and outreach activities in agency and community events.The center operates Monday - Friday, however there may be times when an occasional evening or weekend will be required.

Will need to be able to work independently and as part of a team to promote wellness recovery at the center. Must be able to communicate well verbally even under potentially stressful conditions. Provide presentations to community groups about the services we provide at the Wellness center. Attend all mandatory meetings and trainings as assigned. Meeting with Supervisor weekly for supervision.

Qualifications: BA/BS in social service field preferred, but personal experience can be substituted for formal education. Ideal candidates will have some case management experience, working with adults with mental illness is a plus. Knowledge of local community resources preferred. This individual must be able to have good problem solving skills, be able to work independently but also collaboratively with other agencies and service providers. Knowledge of word processing, ability to compile data for written and oral reports. Experience with Microsoft Office, particularly Excel or other spreadsheets. Familiarity with Google Drive is preferred. Must have reliable transportation, valid driver's license, and insurance.
