Description

Job Summary: The HIV & MAT Community Outreach Specialist works to increase the visibility of the HIV Treatment Prevention and MAT Teams in an effort to promote these services and make them more readily available to members in the community and potential patients. This HIV & MAT Community Outreach Specialist also helps maintain functions of these programs through event planning and coordination, assisting in case management and ensuring that supplies are readily available.

Role & Responsibilities

  • Work cooperatively with Northwest Health Services (NHS) providers and personnel to carry out the goals and
  • objectives of NHS according, to governing body requirements and established internal policies and procedures.
  • Treat others respectfully whether it is a patient, fellow employee, member of management, customer, vendor or
  • visitor to our premises.
  • Exhibit excellent working relations with patients, visitors and staff, effectively communicating NWHS mission.
  • Promote and enhance relationships between community agencies to support identification of patients at risk.
  • Assist with Point of Care testing and outreach events.
  • Work to schedule and coordinate Outreach for MAT and HIV Treatment Services/ HIV testing events or opportunities for community engagement and outreach, organizing testing events, getting the team together and division of tasks
  • Assist to link individuals to treatment and other activities that will improve their health outcomes such as, but not limited to HIV treatment or prevention, primary care services or MAT treatment.
  • Ordering MAT team supplies, HIV test kits, testing supplies (such as alcohol swabs, cotton balls, bandages, lancets etc)
  • Maintaining and preparing consent and risk assessment paperwork
  • Work cooperatively with other clinical personnel assigned to the same patient.
  • Work with marketing department to plan, coordinate and facilitate outreach
  • events to promote HIV prevention, education, and treatment. As well as raise awareness for MAT services and treatment options.
  • Organizing promotional materials and patient resources for each department MAT& HIV
  • Be knowledgeable about community resources appropriate to needs of patients/families.
  • Traveling among local NWHS clinics supporting and educating on specialty services available and how to utilize
  • Attend regular staff meetings, trainings and other meetings as requested.
  • Helping to establish a presence and make it known among our various locations and communities that we offer these MAT and HIV services in the Primary Care setting, thus making it more accessible and encouraging patient involvement and overall buy in.
  • Other duties as assigned.

Qualifications


Education & Experience

  • Possess high school diploma or equivalent.
  • Bachelor’s Degree or Equivalent Experience in Public Relations, Human Services, Health Care
  • Knowledge of some medical terminology preferred.
  • Must possess and maintain a valid Driver’s License, and proof of insurance for personal vehicle used for
  • business.
  • Be able to engage with a diverse population of individuals with respect and without prejudice or judgement.

Required Knowledge, Skills, and Abilities

  • Must be able to talk, see, hear, react, stand, kneel, walk and sit for long periods of time.
  • Possess cognitive skills necessary to read and interpret a variety of instructions furnished in oral or written form.
  • Must demonstrate tact, patience and good communication skills, oral and written.
  • Must respect and maintain confidentiality at all times.
  • Possess excellent organizational skills.
  • Ability to initiate and maintain positive working relationships with staff and other organizations.
  • Maintain a professional appearance and demeanor at all times.
