Job Description
The Licensed Practical Nurse reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
This role is responsible for documenting, and providing care for assigned patients. Utilizing physicians' orders and professional skills the Home Health LPN, under the direction of the RN Case Manager, implements a home health plan that meets each patient's specific needs and is in compliance with JCAHO, federal and state regulations, reimbursement guidelines as well as agency policies and procedures.

Qualifications
Required:
- Completion of a practical nursing program, is licensed in the state where practicing, and who furnishes services under the supervision of a qualified registered nurse.
- BLS certification
- Driver's license
- LIC LPN.
Preferred:
- One year of experience in Home Health.
