COMPENSATION:
- This position comes with a very competitive pay plan. We look at market rates, location, and experience before determining compensation.
SHIFTS:
- 15 hours per week with option to pick up additional shifts
- Patient facing hours and case management responsibilities

Each patient is unique in their needs, so in turn each RN has unique responsibilities in caring for their patient.
Responsibilities
- Patient Care
- Build relationships with patients and families
- Scheduling, prepare for and attend physician renewal/new patient appointments
- Initial Nurse Assessments and start of care visits
- Medication Interaction checks
- Maintain patient binder in home
- Delegates authority appropriately and work to achieve coordination of all aspects of the company programs.
- Ensures implementation of the company policies and procedures.
- Ensures orientation, evaluation and continuing education for all staff.
- Responsible for the overall supervision of all the staff in the area.
Qualifications
- Bachelor of Science in Nursing preferred.
- Current RN licensure in the state of SC required.
- Qualified candidate must possess: strong sense of professionalism, terrific interpersonal skills and be a true team player.
- Previous case management or leadership role(s) which would include; Interviewing, hiring, on-boarding procedures, mentoring and leading.
