Provide emergency assistance to the most vulnerable populations to ensure basic needs are met. Determine eligibility for assistance within multiple program guidelines using available resources and create goals for an individualized client self-sufficiency plan aimed at strengthening and improving client’s well-being. Case management can be short-term or long-term depending on needs, circumstances, and program specifications.
Employee may be assigned to either the Corpus Christi or Robstown office.
The employee is required to be on duty before, during, and after natural disasters and emergencies, as directed by the Department Head.
Include the following:
Interviews clients gathering objective information to make determinations as to appropriate actions.
Provides accommodations to clients as requested or needed.
Addresses any discrepancies or conflicting information and verifies documentation. Gathers and inputs data electronically.
Addresses any immediate risk factors for clients and/or their household members. Provides brief counseling to clients, as needed, including crisis intervention, safety planning and basic needs counseling.
Assesses, evaluates, and determines eligibility for multiple programs based on current guidelines.
Makes independent decisions in discretionary situations or consults with a supervisor in complex or high-risk situations.
Determines the amount of financial assistance to be provided or consults with supervisor in complicated or unique situations. Determines best funding source based on current guidelines and availability.
Develops an action plan to help clients meet their emergency need and create goals for self-sufficiency, including basic counseling and problem-solving. Monitors client progress and follows-up regularly to ensure services are timely
Provides client with information and make referrals to other community resources. Maintains a working knowledge of other community social service, health, employment, and education resources.
Communicates effectively with clients, other agencies, the public, officials, and other county employees.
May be asked to participate in community outreach activities.
Advocates on behalf of clients to obtain appropriate services.
Manages an active caseload, including home visits within the service area. Ongoing long-term case management for specific clients. Conducts regular reviews for continued eligibility.
Maintains records in an accurate and current manner.
Prepares monthly statistical and/or disability reports on caseload.
If licensed by a professional board, must comply with licensure requirements and code of conducts and/or code of ethics.
Performs other duties that may be assigned.
1) a bachelor’s or master’s degree in social work or a behavioral science. A social work degree is preferred. One year of experience in case management is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS: A valid Texas Driver’s license is required. If licensed by any professional board, verification of licensure must be provided.
Licensure from Texas State Board of Social Worker Examiners (LBSW, LMSW, LCSW) under the Texas Behavioral Health Executive Council (BHEC) is preferred.
Ability to communicate effectively orally and in writing. Excellent writing skills, with the ability to write in a detailed and concise manner and writing free of grammatical errors.
Ability to conduct virtual interviews, create digital client files and effectively manage a digital caseload.
Requires the ability to explain rules, regulations, and policies of Department programs in terms understood by clients and the public.
Requires the ability to exercise considerable tact, courtesy, discretion, firmness, and fairness in contact with applicants, clients, and the public.
Demonstrate sound judgment in the effective and efficient use of human and financial resources. Ability to use modern office equipment including computers.
Requires the ability to maintain confidentiality based on relevant laws, policies and/or procedures.
Requires the ability to work under pressure from frequent interruptions.
Ability to deal with clients in crisis situations or experiencing stress.
Knowledge of interviewing techniques.
Capability to provide brief counseling services.
Ability to be culturally sensitive when working with a diverse population with the goal of being culturally competent.
Ability to complete work within specified deadlines.
Bilingual ability (English/Spanish) is preferred.
