Tri-County Health Department- Full time, Westminster Office (Serving Adams County) What we do: Promote, protect and improve the lifelong health of individuals and communities in Adams, Arapahoe and Douglas Counties through the effective use of data, evidence-based prevention strategies, leadership, advocacy, partnerships and the pursuit of health equity. In short, we strive to provide optimal health across the lifespan for the populations we serve.

Nurse-Family Partnership (NFP) is a national evidence based maternal, infant and early childhood home visitation program, which provides communities the opportunity to foster long-term success for pregnant and parenting clients, their children and families. This position provides comprehensive, professional public health nursing services for low income, vulnerable, first time pregnant women and their families, from early in pregnancy to the child’s second birthday. Primary goals of the program include improved pregnancy outcomes, optimal child health and development, positive parenting and family self- sufficiency.

The NFP nurse supports and promotes health through delivery of client centered, intensive family support (home visitation) and case management services. Each nurse home visitor is responsible for recruiting and maintaining a caseload of 25 families, maintaining fidelity to the program model, meeting program goals and objectives, as well as supporting client centered goals, through health assessments and targeted public health nursing interventions.

While this position is based in Adams County, it typically requires extensive travel throughout Adams, Arapahoe and Douglas Counties and to other locations in the Denver Metro area. As of March 2020, all NFP services are being delivered via telehealth due to COVID-19 considerations, therefore travel expectations have been temporarily reduced. Some travel is still required. Staff may be required to work at their designated agency office location and will need a HIPAA secure home location which complies with agency telework expectations.

Requirements:

• Bachelor’s Degree in Nursing from an accredited college or university.

• Minimum of one-year nursing experience, preferably in the area of Maternal Child Health and community-based services. Graduate nurses and those with less than the required experience may be considered at an entry level Public Health Nurse position until the minimum year of nursing experience is met.

• Active Colorado Nursing license and current CPR certification

• Valid Colorado Driver’s License

• Access to reliable vehicle and automobile insurance.

• Comfortable with regular use of an automobile, travelling and working in the community, with unpredictable weather and less than optimal environmental conditions at times (between 80%- 90% of the time; pre-COVID-19).

• Fluent Spanish speaking, highly preferred.

• The ideal candidate will be able to quickly develop therapeutic alliances with a broad range of diverse clients, demonstrate flexibility, adaptability and creativity in delivery of nursing care, employ strong computer and technology skills and thrive in both traditional and virtual team environments.

• Due to the relationship- based nature of this position, we are looking for a long-term fit.
