What are the responsibilities and job description for the Case Manager position at Nystrom & Associates, Ltd.?
As a Case Manager on Nystrom & Associates’ Behavioral Health Home (BHH) team, you will be part of a community based team providing case management services for individuals with a mental health diagnosis. You’ll make a major impact on your local community by helping individuals get connected to mental health services, preventative care, and community programs.
Behavioral Health Home (BHH) is a lower level of care and considered a step-down from Adult Rehabilitative Mental Health Services (ARMHS), Targeted Case Management, or Assertive Community Treatment (ACT). BHH is a team-based model that includes a case manager, nurse and a community health worker.
The Case Manager, Community Health Worker, and Nurse work as a team, and will work with clients on goals related to:
- Connection to mental health services, recovery support services, preventative care, and community programs such as housing, food shelves, employment, etc.
- Transitions in care
As a Case Manager, you’ll have the opportunity to:
- Provide culturally responsive, holistic care in the community.
- Work closely on a team with nurses and case managers.
- Ensure that the client is aware of resources and are supported in efforts to access resources in order to address the client’s identified goals and needs (i.e., county social services, housing, employment).
- Assist as needed in conducting the initial needs assessment and developing the preliminary action plan.
- Provide clients with health promotion services by assisting them with skills related to overall health and wellness
- Interact with clients on the phone or in-person monthly.
Requirements:
In order to be considered for this position, you must be qualified in at least one of the following ways:
- Hold a Bachelor’s or Master’s degree in behavioral sciences or related field & have completed an internship or practicum with adults OR
- 30 semester hours in behavioral sciences or related field & 2000 hours of supervised experience working with adults OR
- 4000 hours of supervised experience working with adults with mental illness or substance use disorder.
Additionally, you must meet the following requirements:
- Desire to work with Adults diagnosed with a mental health diagnosis in an in-home setting
- Knowledgeable of community resources and health interventions
- Valid Driver’s License, Auto Insurance, and Reliable Vehicle; access to the internet
