Job Title: Executive Director, Oakland Home Care
Position Summary: Oakland Home Care provides home care services to include Physical Therapy, Occupational Therapy, Speech and Language Pathology, Medical Social Work, and Skilled Nursing to homebound patients. We primarily serve residents living in luxury senior living communities (Independent Living, and Licensed Assisted Living and Memory Care) in the suburban areas of Oakland, Macomb, and Wayne counties.
The Executive Director (ED) is responsible for the overall management and direction of the agency. They plan, direct, and coordinate operational and clinical activities at the highest level of management with the help of clinical leadership and the governing body. She/he supervises home health services to homebound patients in their place of residence in accordance with attending physician orders and plans of care. The ED thrives on providing a high level of customer service and leading a strong team consisting of administrative support staff, clinical leadership, and clinical staff. It is the responsibility of the ED to coordinate all of the moving parts of the agency to deliver the best possible outcomes to all of its patients. This is a unique opportunity to lead and develop an agency to provide high-quality care and positive patient outcomes. The role will be based out of our office in West Bloomfield and may include occasional travel to the communities we serve.
Main Job Tasks and Responsibilities:
  • Oversee the clinical aspects of the agency, ensuring our high standards, including those of our accrediting bodies, Medicare, Medicaid, as well as any state or federal guidelines are being met. All to ensure our patients receive the highest quality outcomes.
  • Assesses and implement appropriate staffing of the agency based on patient census. Is responsible for recruiting, hiring, evaluating and terminating staff. Assists with the orientation and training of new and tenured staff.
  • Serves on utilization review committee, oversees ongoing performance improvement program, and coordinates services with other community agencies as needed.
  • Observes the utmost confidentiality and safeguards all patient related information, including Protected Healthcare Information (PHI).
  • Develops excellent relationships, communicates effectively and professionally with senior living community leadership, physicians and physician groups to maintain and grow census.
  • Investigates any problem relating to patient care or conditions which might harm the patient and/or employee well-being. Reports these issues to the Executive Director and/or governing body.
  • Develops and adheres to annual budget while determining appropriate use of resources.
  • Conducts patient case conferences, in-services, staff meetings and maintains appropriate documentation as necessary.
  • Is normally available at all times during and after operating hours (on call); may designate a qualified temporary replacement if he/she will not be available.
  • Other duties as assigned by the governing body.
Job Requirements:
  • Maintains a clinical license in good standing with appropriate/accredited degree based on the clinical discipline.
  • Prefer a Registered Nurse, Physical Therapist, or Occupational Therapist.
  • In lieu of clinical licensure, candidate must possess a Bachelor's degree AND a minimum of 2 years of home healthcare management experience or related health program.
  • Two to five years of experience in health care/home care.
  • Working knowledge of both Medicare Part A and Part B.
  • Two years’ experience in a supervisory position, preferred.
  • Proficiency with computer applications, such as electronic medical records (Kinser preferred) and Microsoft office products.
  • Knowledge and ability to apply community health principles and practices.
  • Ability to supervise, guide and develop skills and performance of service personnel.
  • Ability to exercise independent judgment.
  • Ability to enlist cooperation of many people in furthering a program.
  • Ability to deal effectively with high levels of stress.
Physical Requirements:
  • Constant use of manual dexterity, auditory and visual skills and gross motor skills with frequent use of bi-manual dexterity and fine motor skills.
  • Ability to follow written and oral instructions and procedures.
  • Ability to reach, bend, twist, squat, kneel, push and pull.
  • Ability to lift/carry up to 50 pounds. 80 pounds of pressure may be placed on the associate through transfers using transfer belts or mechanical assists.
  • Ability to perform repetitive movements including simple grasping, pushing, pulling and fine manipulation.
  • Constant standing and walking.
