Programming Coordinator - Children's Residential Services

Our programs service adults, children, and families. Your role will be essential to our success, and your efforts will influence our ability to provide treatment to our community.

Schedule: Five days a week, late afternoon and some evenings required. On-site across seven locations.

Competencies: 

The ideal candidate must be able to demonstrate the ability to provide supportive services for youth. Organized and must demonstrate the ability to maintain contact with the youth, family and their support system. Must demonstrate the ability to produce a progress note in the CYBER and agency HER systems. Must demonstrate the ability to generate and maintain appropriate documentation as noted by chart review with at least 98% accuracy. Must demonstrate the ability to access local networks of services as dictated by the current ICP. Must demonstrate the ability to maintain a professional relationship with youth, family, their support system and all service providers. Must participate in and pass all mandatory trainings as scheduled. Must demonstrate basic computer knowledge.

Responsibilities:

• Receive referrals from the CSA through a “warm line” at the office
• Maintain caseload and monitor stabilization services
• Work with the Supervisor and other team members to coordinate care for all clients served including follow-up and collateral contact
• Maintain documentation with 98% accuracy, and within the required CSOC/Oaks time frame
• Participate in team meetings and staff meetings
• Attend all required trainings as scheduled

Qualifications:

• Bachelor's degree (BA/BS) in a behavioral health or related human services field, such as social work, counseling or psychology and at least four to six months of experience working with youth. Knowledge of the Nurtured Heart Approach preferred.

• Valid Driver's License required.
