Position Summary: This position oversees the quality management and oversight of clinical services offered through the outpatient treatment clinic. The ideal candidate will have a strong background in geriatric care, quality management, and a passion for improving healthcare outcomes for seniors.
Key Responsibilities:
- Exemplify OCC’s core values while ensuring continuous innovation and development in methodologies and interventions to delay functional decline and reduce the challenging behaviors that trigger institutionalization by offering hands-on education and support to enhance quality of life, understand dementia, reduce challenging behaviors, promote function, communicate effectively, make the home safer, and optimize independence.
- Team Oversight and Training: Supervise the outpatient treatment clinic team, including hiring, training, and performance management. Ensure staff are well-trained and equipped with the necessary skills to deliver high-quality care. Facilitate ongoing professional development opportunities and coaching sessions.
- Quality Management: Develop, implement, and monitor quality assurance programs and clinical performance metrics to ensure compliance with regulatory standards and best practices and assess the performance of clinical services.
- Clinical Oversight: Oversee the delivery of clinical services, ensuring they meet the needs of patients and are aligned with the clinic’s mission, vision, contractual and regulatory guidelines.
- Provide clinical oversight and direction for the CMS GUIDE model pilot program and other program initiatives.
- Conduct physical assessments, review medications for appropriateness and polypharmacy, provide external lab referrals for comprehensive testing, and educate on managing common health concerns and advocating for medical care.
- Data Analysis: Utilize data analytics to track performance, identify areas for improvement, and report on the effectiveness of clinical services and quality management initiatives. Lead initiatives aimed at improving patient outcomes, including the implementation of evidence-based practices and innovative healthcare solutions.
- Stakeholder Engagement: Collaborate with healthcare providers, staff, and stakeholders to ensure best practices and evidence-based care. Act as a liaison between the clinic, patients, families, and external healthcare providers to foster a collaborative approach to patient care.
- Program Development: Collaborate with medical staff to design and enhance clinical programs and participate in strategic planning and decision-making processes.
- Develop and facilitate healthy living and resilience program as identified by the ACL grant if awarded
- Provide education and training to staff on clinical best practices and quality improvement initiatives.
- Act as a liaison between Oakwood Creative Care and external healthcare partners.
- Report on the progress and outcomes of clinical programs and other program initiatives to the leadership team.
- Comply with contractual grant, state, and federal requirements.
- Participate in additional opportunities for growth and development.
- Serve as an ambassador and supporter of OCC fundraising initiatives.
- Participate in ongoing professional development through continuing education, workshops, and conferences to stay updated on the latest research and best practices in gerontology and Dementia best practices.
- Collaborate with Care Navigators and Day Clubs to provide support and guidance on cases needing physician assistance
- The Medical Director will provide clinical, coding, and reimbursement expertise as well as directing case management when necessary.
- Participate in the development and review of Medical and Clinical policies as assigned
- Support the organization as a subject matter expert
- Other requirements as determined by the President and CEO
Qualifications:
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board certification in Geriatrics, Family Medicine, or Internal Medicine.
- Active medical license in the state of AZ.
- Minimum of 5 years of clinical experience, with a focus on geriatric care preferred.
- Experience in quality management and performance improvement initiatives.
- Strong leadership, communication, and interpersonal skills.
- Ability to work collaboratively with a multidisciplinary team.
- Knowledge of CMS guidelines and regulatory requirements.
Position Details:
- Part-time position (approximately 5 -10 hours per week)
