Position Description: Under the supervision of the Clinical Director, the Assessor performs a variety of clinical and administrative work when it has been determined through an initial screening that the youth has mental health or addictive disease concerns. The initial Behavioral Health Assessment is required within the first 5 days of service, with ongoing assessments completed as demanded by individual client need and/or by service policy. The Behavioral Health Assessment process consists of a face-to-face or telemed comprehensive clinical assessment with the individual, family member/responsible caregiver(s), and others significant in the youth’s life as well as interviews with collateral agencies/treatment providers/relevant individuals. This position includes data gathering and formulation of an Individual Resiliency Plan/Treatment Plan which are all needed before the provision of mental health services to children and Adolescents in need of treatment and support. The purpose of the Assessment process is to perform a formalized assessment in order to determine the youth’s problems, strengths, needs, abilities and preferences, to develop a social (extent of natural supports and community integration) and medical history, to determine functional level and degree of ability versus disability, and to develop or review collateral assessment information. The information gathered should support the determination of a differential diagnosis and assist in screening for/ruling-out potential co-occurring disorders. This position requires a broad knowledge of therapeutic processes and methodologies.
Duties and Responsibilities:
1) Receives referrals of clients in need of mental health services.
2) Conduct assessments in office and out of office.
3) Schedules, conducts, and completes a face to face assessment of client’s needs within the specified time frame i.e. calls to schedule assessment within 24 hours of accepting assignment. If client cannot be reached after two days Assessor reaches out to Oasis Office for help in reaching the family. Legals, BHA and Safety Plan is completed and submitted to identified Oasis Staff within 72 hours of the intake date.
4) Ensures the orientation packet/leagals are completed thoroughly with every necessary signature.
5) Completes the Behavioral Health Assessment and e-mails to designated Oasis Staff for approval.
6) Completes the Safety Plan and gets client and family signatures.
7) Completes and submits the CANS
8) Enters a BIRP note into Electronic Note system for the legals, safety plan and assessment (H0031…2 hours)
9) Make any noted corrections on BHA and print and sign.
Minimum Qualifications:
1. Must have a Master’s degree in social work, clinical psychology, counseling or a closely related field and actively working toward licensure.
2. Experience completing assessments and Individual Resiliency Plans preferred.
3. Demonstrated knowledge of mental health disorders and treatment methods.
4. Ability to establish and maintain working relationships with consumers, families, service providers and colleagues.
5. Knowledge of clinical operations and procedures, Medicaid services, and community mental health resources.
6. Strong written and verbal communication skills.
7. Exceptional verbal and written communications skills.

Pay: $35.00 - $50.00 per hour
