On Demand Counseling of Newton Falls Ohio is looking to hire full time case managers to join our growing team! The case manager position will be utilized to facilitate linkages between agencies, actively work with clients to determine treatment needs, coordinate with treatment team to come up with plan of care, work within client health record to document services. This position has room to work within employee strengths. Some interests of our agency are in financial counseling, wellness coaching, and nutritional education, among others. Must be able to develop working relationships with our clients and show them that we truly care about their wellbeing. The case manager will work very closely with the rest of the clinical and medical treatment team to aid clients in a variety of needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision and values of On Demand.
- Make referrals for other mental health / higher level of care.
- Act as an advocate for program participants with courts, schools, treatment centers, and other services as appropriate in order to make referrals and conduct follow-ups.
- Provide individual counseling, engage clients, family sessions (if asked).
- Provide outreach and case management to primary clients.
- Prepare written reports, regular case dictation and summaries for consultation according to program standards and professional ethics.
- Participate in curriculum development and supervision of clients.
- Participates in staffing meetings.
- Maintain client engagement and reengagement in services.
- Provide group cofacilitation as needed.
REQUIRED QUALIFICATIONS:
- Must have valid licensure in the state of Ohio, including but not limited to CDCA, LCDC II, LSW, etc.
- Must have knowledge and experience working with alcohol/drug assessment and case management.
- Must be able to form good working and therapeutic relationships.
- Must be organized, detail oriented and the ability to maintain confidentiality.
- Must possess group work skills.
- Must have competent oral, written and interpersonal communication skills.
- Good working knowledge of computers and familiarity of Microsoft Office.
- Experience working with a team of professional staff desirable.
