Currently seeking a skilled and driven Social Worker to welcome our ever-growing team!
Our goal for The Rehab of K C South is to provide our residents with quality care. We desire individuals who have a passion for helping people, who thrive our team environment, and who want to make a difference all while advancing their skills.
Responsibilities:
- Maintain a community resource list
- Complete initial assessments
- Implement care plans
- Ensure regulatory compliance
- Complete MDS sections as assigned, ensure all areas are addressed in the
care plan as necessary.
Requirements:
- A bachelor's degree in social work or a closely related field
- A master's degree a plus
- State licensure preferred (CSW, ACSW, LSW, LCSW)
- Interpersonal, critical thinking, and organizational skills
