PRN Weekend (Saturday/Sunday), Days (Hours vary).
The Case Manager is a licensed nurse who provides ongoing and comprehensive assessment, planning, monitoring, implementation and evaluation of the plan of care. The Case Manager enhances the quality of patient management and satisfaction, promoting continuity of care and cost effectiveness through integrating functions of case management and utilization review. He/she acts as a resource, advocate, and a liaison for the patients on his/her caseload by monitoring the healthcare process and intervening to promote high quality, cost-effective healthcare within the framework of the patient’s rights. He/she assesses the reviewed population of patients for issues that may negatively affect length of stay or quality of care. The Case Manager initiates discharge planning at admission and utilizes all available resources in an appropriate manner. Assesses, coordinates, plans, monitors, implements, and evaluates the plan of care.
Education and Formal Training: Graduate of an accredited registered nurse program.
Work Experience: Minimum of 2 years of Medical/Surgical, Intensive Care, Emergency Room, Hospice or Home Health experience. Management experience helpful. Utilization Review/Case Management experience preferred.
Knowledge, Skills, and Abilities Required: Demonstrates sensitivity, sound judgment, and a professional attitude when relating with patients, families, and co-workers at all times. Actively functions as a patient advocate. Must have the ability to learn and demonstrate knowledge of Utilization Review/Case Management processes. Has knowledge of the current hospital emergency policies and procedures, fire, safety, disaster, and infection control policies.
