Opportunity Community Services, Inc works under a 245D license and provides ICS, ES, and IHS services.
The Community Integrative Specialist position oversees several categories of service; Integrated Community Supports (ICS), Employment Services (ES), and Individualized Home Supports (IHS). ICS is a service that provides support and training in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting. ICS can be delivered up to 24 hours per day in the person’s living unit or in the community. Employment Services that help people find and retain paid employment in community businesses where people with limited abilities are employed. The program is offered in three different methods: Employment Exploration Services (Exploration), Employment Development Services (Development), and Employment Support Services (Support). Individualized Home Supports are designed to help people to live well and stay safe in their own home. This service provides training, assistance, support and/or guidance with things like money management, meal planning and nutrition, household chores & other personal needs. You will be responsible for managing a caseload of approximately 10 clients. Candidate must be able to provide individualized services and support to help people maintain paid employment in community businesses/settings. This position works both in-person with the client and remotely in integrated community settings.

Duties/responsibilities include
- Act as an advocate for the Client
- Participate in Client team meetings
- Schedule sessions with clients
- Complete and document all client sessions in Pavillio
- Complete mileage sheets
- Communicate with outside supports (such as Waiver Case Manager, County Workers, etc.) to best serve client as needed
- Complete individualized, strengths-based assessments, employment opportunity discovery strategies and skills training for Client to find and succeed in a job
- Work with individuals with a variety of disabilities including elderly, developmentally disabled and mental health
- Reviewing options to access community resources and learning to be aware of crosswalk safety
- Teaching how to safely navigate the internet and social media
- Assist client with scheduling medical or therapy appointments
- Encouraging and guiding clients to improve physical and mental health
- Reminding clients of fire safety and proper usage of stoves
- Helping client to create a budget, determine financial priorities, meal planning, creating shopping lists, and cooking together
- Supporting the person with figuring out how to address a disruptive neighbor and report the concern to the landlord
- Reviewing strategies to prevent a crisis and helping the person create a plan of who they can contact and what they can do in the case of a crisis
- Supporting the person through a difficult time using their identified person-centered positive support strategies
- Working together to schedule a Metro Mobility ride or practicing taking public transit together
- Helping someone create and follow an exercise routine or a plan for engaging in another coping skill (yoga, journaling, etc.)
- Brainstorming what to do if someone is pushing a boundary they have set
- Identifying groups, the person can connect with (clubs, community ed, religious communities, etc.)
- Complete all required paperwork/documentation under 245D and 245A
- Travel throughout several counties to meet Clients in the community
Required Skills
- Two-year degree in a human/social services field or five years of experience in a disability related position.
- Must be at least 18 years old, have experience working with people with disabilities and disability issues.
- Must have a Driver's License and a reliable vehicle with active automotive insurance
