PHC Intake Coordinator

We are looking for a motivated PHC Coordinator to organize and coordinate client care, administration duties and office procedures. Your role is to create and maintain a pleasant work environment, ensuring high levels of organizational effectiveness, communication and safety. PHC Coordinator duties and responsibilities include coordination of care, scheduling meetings and appointments, making office supplies arrangements, greeting visitors and providing general administrative support to our clients/employees. A successful Coordinator should have experience with a variety of office software (HHA or Vesta, email tools, spreadsheets and databases) and be able to accurately handle administrative duties. The coordinator should be able to ensure the smooth running of the office environment as well as to improve the company’s procedures and day-to-day operations. Additional duties and responsibilities includes:

• Communicates with clients, attendants and field supervisor to coordinate the initiation of services, changes in schedules, suspension of services, etc.
• Notifies case worker and MCO service coordinators of the initiation of services, delay in service interruptions and service suspensions via 2067.
• Has knowledge of the PHC program rules.
• Assist the attendant supervisor in the documentation of changes to the ISP and communication with the case workers and service coordinators.
• Prepares documentation packets to document decreases and changes in tasks in accordance with agency policies.
• Communicates to coordinate services with the attendant and client followed by documentation to identify and report allegations received by phone to supervisor immediately.
• Maintains agency logs updated in accordance with agency policy.
• Maintains cooperative communication with EVV and billing staff.
• Provides ongoing education and training related to EVV.

Pay: From $12.00 per hour

Schedule:

• 8 hour shift
• Day shift
• Monday to Friday

Experience:

• Office management: 1 year (Preferred)
• Home health/PHC: 3 years (Required)
